Дисертаційна робота присвячена обґрунтуванню теоретико-методичних засад та розробці практичних рекомендацій щодо удосконалення функціонування організаційно-економічного механізму розвитку сфери охорони здоров’я в Україні. У дисертації поглиблено теоретичні положення функціонування сфери охорони здоров’я, у частині уточнення сутності такої сфери. Запропоновано її зміст розглядати як систему заходів та дій соціального характеру, метою яких є збереження та відновлення здоров’я громадян, а також профілактика захворювань в довгостроковій перспективі за рахунок надання своєчасних та якісних медичних послуг. Також досліджено особливості еволюційного розвитку сфери охорони здоров’я в Україні. Це дало можливість доповнити існуючі підходи до конкретизації етапів такого розвитку наступними: заснування Національної служби охорони здоров’я, трансформація організаційно-економічного механізму розвитку сфери охорони здоров’я, який пов'язаний з підвищенням соціальної відповідальності як бізнесу, так і громадян, та майбутнім впровадженням медичного страхуванням. У роботі поглиблено понятійний апарат теорії державного регулювання економіки у частині уточнення змісту категорії «організаційно-економічний механізм розвитку сфери охорони здоров’я». Зокрема такий механізм запропоновано розглядати як сукупність взаємопов’язаних принципів, функцій, методів та інструментів, за допомогою яких формуються оптимальні умови для використання наявних х ресурсів у держави та громадян з метою забезпечення ефективного функціонування та розвитку сфери охорони здоров’я на інноваційних засадах з урахуванням довгострокових перспектив. Також головними завданнями функціонування зазначеного механізму визначено наступні: створення системи гарантій для населення; впровадження інноваційного фінансового інструментарію забезпечення функціонування сфери охорони здоров’я; оптимізація ресурсів медичної сфери; створення вільного ринку надання медичних послуг з метою підвищення конкурентоспроможності медичних закладів різної форми власності; запровадження прозорих процедур надання медичних послуг; формування культури здорового способу життя. Поглиблено теоретико-методичні положення функціонування організаційно-економічного механізму розвитку сфери охорони здоров’я в частині обґрунтування його мети, функцій, принципів, методів та відповідного інструментарію, що сприятиме підвищенню ефективності роботи сфери охорони здоров’я та забезпечить досягнення головної її мети – надання доступних медичних послуг високої якості незалежно від фінансових можливостей пацієнтів та соціально-економічного становища держави. Обґрунтовано базові методологічні положення, на яких відбувається формування відносин суспільства та держави у сфері охорони здоров’я. Визначено, що такі положення у сучасних реаліях повинні базуватися на: активізації профілактичної політики щодо базових захворювань громадян; проведенні вчасних компаній щеплень; високій соціальній відповідальності окремого громадянина; орієнтації на збереження здоров’я людини; вдосконаленні інституту сім’ї, охорона материнства, батьківства й дитинства. У дисертації визначено, що для реалізації окресленого стратегічними орієнтирами розвитку сфери охорони здоров’я мають стати якість і доступність медичних послуг, використання сучасних технологій у сфері медичного обслуговування, державна підтримка розвитку науково-технічного прогресу в медичній сфері, збереження та розвиток потенціалу закладів охорони здоров’я незалежно від форми власності. У дисертації поглиблено науково-концептуальні положення функціонування сфери охорони здоров’я, що реалізовано через детальний розгляд адаптаційного механізму її розвитку з визначенням змісту такого механізму, його мети, методів, функцій та принципів, що дало можливість обґрунтувати сучасні напрямки подальшого розвитку зазначеної сфери з урахуванням об’єктивних причин необхідності здійснення її ендогенної адаптації (врахування внутрішніх детермінантів функціонування системи медичних закладів) та екзогенної адаптації (оцінка та врахування особливостей сформованого зовнішнього середовища, в якому розвиваються медичні установи). На основі детального аналізу сучасних тенденцій розвитку сфери охорони в Україні було ідентифіковано сукупність системних проблем у функціонуванні організаційно-економічного механізму розвитку сфери охорони здоров’я. Це було здійснено на основі застосування методики SWOT-аналізу і дало можливість конкретизувати потенційні можливості розбудови зазначеної сфери та сформулювати прикладні положення забезпечення їх використання у подальшому.^UThe dissertation work is devoted to the substantiation of the theoretical and methodological principles and the development of practical recommendations for improving the functioning of the organizational-economic mechanism of the healthcare sector development in Ukraine. In the dissertation, the theoretical provisions of the healthcare sector functioning in terms of clarifying the essence of this sphere are deepened. It is proposed to consider its content as a system of measures and actions of a social nature, the purpose of which is to preserve and restore the health of citizens, as well as to prevent the diseases in the long term due to the provision of timely and high-quality medical services. The peculiarities of evolutionary development of the healthcare sector in Ukraine were also investigated. This made it possible to supplement the existing approaches to the specification of the stages of this development with the following ones: establishment of the National Healthcare Service, transformation of the organizational-economic mechanism of the healthcare sector development, which is related to the increase of social responsibility of both business and citizens, and the future introduction of health insurance. In the paper, the conceptual apparatus of the theory of state regulation of the economy in terms of clarifying the content of the category “organizational-economic mechanism for the healthcare sector development” is deepened. In particular, this mechanism is proposed to be considered as a set of interrelated principles, functions, methods and tools, with the help of which the optimal conditions are formed for the use of available resources of the state and citizens to ensure effective functioning and development of the healthcare sector development on an appropriate basis, taking into account the long-term perspectives. Also, the following are defined as the main tasks of the functioning of the specified mechanism: creation of a system of guarantees for population; introduction of an innovation toolkit to provide the functioning of the healthcare sector; optimization of the resources of the medical sphere; creation of a free market for the provision of medical services to increase the competitiveness of medical institutions of various forms of the ownership; introduction of transparent procedures for the provision of medical services; formation of a healthy life culture. The theoretical and methodological provisions of the functioning of the organizational-economic mechanism of the healthcare sector development have been deepened in terms of substantiating its purpose, functions, principles, methods and appropriate tools, which will contribute to increasing the effective work of the healthcare sector and ensure the achievement of its main goal – providing affordable high-quality medical services regardless of the financial capabilities of the patients and the socio-economic situation of the state. The basic methodological provisions on which the relations between the society and the state are formed in the healthcare sector are substantiated. It was determined that such provisions in modern realities should be based on the following: activation of the preventive policy regarding the basic diseases of citizens; conducting timely vaccination campaigns; high social responsibility of an individual citizen; orientation to the preservation of the human health; improvement of the institution of the family, protection of motherhood, fatherhood and childhood. In the dissertation, it is determined that in order to implement the outlined, there must be strategic guidelines for the healthcare sector development, the quality and availability of medical services, the use of modern technologies in the healthcare sector, state support for the development of the scientific and technical progress in the medical sector, preservation and development of the potential of healthcare institutions regardless of their ownership form. In the dissertation, the scientific-conceptual provisions of the healthcare sector functioning are deepened, which was implemented through a detailed consideration of the adaptation mechanism of its development with the determination of the content of this mechanism, its purpose, methods, functions and principles, which made it possible to substantiate the modern directions of further development of the specified sector, taking into account the objective reasons for the need of its endogenous adaptation (taking into account the internal determinants of the functioning of the system of medical institutions) and exogenous adaptation (assessment and consideration of the features of the formed external environment in which medical institutions develop). Based on the detailed analysis of the modern trends in the healthcare sector development in Ukraine, a set of systematic problems was identified in the functioning of the organizational-economic mechanism of the healthcare sector development.
