Продовжено вивчення клінічного перебігу хронічного гастриту на фоні цукрового діабету 2 типу. Вперше визначені клініко-патогенетичні особливості перебігу хронічного гастриту у хворих на цукровий діабет 2 типу у порівнянні з хворими на цукровий діабет 1 типу та особами без супутнього цукрового діабету з урахуванням їх H.pylori-статусу та наявності синдрому надмірного бактеріального росту.Вперше виявлено, що на спектр гастроінтестинальних симптомів у хворих на хронічний гастрит у поєднанні з цукровим діабетом 2 типу статистично достовірно впливає інфекція H.pylori та синдром надмірного бактеріального росту. У дисертаційній роботі продемонстровано, що поєднання таких факторів,як цукровий діабет 2 типу та інфікування H.pylori, здатні потенціювати дисбаланс у системах оксиду азоту, про- та антиоксидантного захисту, суттєво послаблювати резистентність слизового бар'єру шлунку, а також викликатиметаболічну ендогенну інтоксикацію. The peculiarities of chronic gastritis pathogenesis in patients with diabetes mellitus type 2were studied and compared to those with type 1 diabetes mellitus and subjects with no signs of carbohydrate metabolism disturbances regarding their H.pylori-status and presence of small intestinal bacterial overgrowth.Іt was established for the first time that clinical spectrum of gastrointestinal symptoms in patients with chronic gastritis and diabetes mellitus type 2 is significantly defined by H.pylori-infection and small intestinal bacterial overgrowth. The research shows that combination of such factors, as diabetes mellitus type 2 and H.pylori infection may potentiate the imbalance in nitric oxide system, weaken the antioxidant defense system, decrease resistance of stomach mucosal barrier, induce metabolicendogenous intoxication. We improved for the first time that association of chronic gastritis and diabetes mellitus type 2 leads to decreased quality of life of patients, not only through psychoemotional state worsening but also through marked physical state deterioration. The substantiation of eupatilin prescription was further developed in patients with H.pylori-associated chronic gastritis and diabetes mellitus type 2. The option for antihelicobacter therapy optimization was proposed with the addition of probiotic and eupatilin. Such combination of drugs allows to affect pathogenetic mechanisms of stomach mucosal injury in patients with H.pylori-associated chronic gastritis and diabetes mellitus type 2.
